Clinic

Your son or daughter should have brought home the Orange County Public School Emergency Student Information Form. This form needs to be completed and signed at the bottom in ink. It is extremely important that the information is correct. Please be sure the parent/guardian names and phone numbers are listed and correct. If you wish other person(s) to be called in case you are unavailable or give permission for them to pick up your child please write these names in the appropriate space. Just a reminder - PLEASE SIGN THE FORM IN INK. Students should return these forms to the school as soon as possible.

Medications of any kind must be brought to school by an adult. Students may not carry any medication. If your child uses an inhaler, he or she may carry it as long as the proper form is completed and on file in the clinic. If this is the case, please have your child come to the clinic to pick up an authorization for inhaler use form.

If you have any concerns or questions, please feel free to call. The clinic number is 407 521 2360 ext. 2254. A nurse is usually in the building from 9:00 am to 3:30 pm. If your child is having difficulty or situations at home are changing, please call and let us know. Good communication is a key to good attendance which in turn helps to develop good academics.

Medication

If your child is taking medication that is to be administered at school, it MUST be kept in its original container, with a label affixed by the physician or pharmacist, showing the child's name, the dosage, and the schedule for administration. The parent or guardian must complete an "Authorization for Medication" form for all medication that will be distributed by Gotha personnel. The number of pills or tablets brought to school will be counted by the nurse and the person bringing the medication. The count will be noted on the form. When medication is dispensed, the student will be asked to sign a log noting date, time, and dosage. It is the student's responsibility to remember his or her medication schedule. Students are not permitted to carry medications with them whether they are prescription or over-the-counter drugs. The only exception is for students who must carry inhalers because of Asthma.
