Shots, Shots, Shots!
All 7th grade students must receive the Hepatitis B series, a Tdap (Tetanus, Diphtheria, Pertussis), 2 doses of varicella, and 2 doses Measles Vaccine (MMR). It takes a minimum of 4 months to complete the Hepatitis B series. Remember that the school office will need a copy of the 680 form before students receive their 7th grade schedule. Please do not wait any longer to bring it in.
** Any students without or missing required immunizations will not receive a schedule.
Students who live within the Gotha Middle School zone and complete 5th grade in an Orange County public school or an OCPS approved charter school are automatically enrolled.
In-County Transfers
Proof of residency; Lease, deed, homestead exemption, and current utility bill
Guardianship; must show proof of guardianship if not guardian at previous school
Transfer or withdrawal grades from previous school
Out of County / Private School
Proof of residency; Lease, deed, homestead exemption, and current utility bill
Birth certificate, passport, baptismal record etc...
Immunization: Form 680 (can be obtained from Orange County Health Department) or Form 3040 (can be obtained from any doctor or health department after receiving a physical) All out of county/ private school students MUST have a recent physical (within the last 12 months)
Guardianship: must show proof of guardianship if not natural parents
Transfer or withdrawal grades from previous school
Out of State / Country

- Declaration of Domicile (for new residence of Florida)
- Birth certificate, passport, baptismal record
- Immunization: Form 680 (can be obtained from Orange County Health Department) or Form 3040 (can be obtained from any doctor or health department after receiving a physical). All out of state / country students MUST have a recent physical (within the last 12 months)
- Guardianship: must show proof of guardianship if not natural parents
- Transfer or withdrawal grades from previous school
