Updated OCPS Sports Physical Packet 2022-2023!!

  • Each student that participates in athletics needs to have the OCPS SPORTS PHYSICALPACKET completely filled out by a parent/guardian and a Doctor prior to try outs. School Physicals are not the same as a sports physical. A sports physical is good for one calendar year from date of the physical. Middle School Level has OPTIONAL EKG Exam for this calendar year.

Be sure to turn your completed packet in to the MAIN OFFICE by the deadline listed below. There will be NO EXCEPTIONS to this policy. Athletes who turned in their completed/incomplete packet past the deadline will not be allowed to participate in that sport.


 Soccer- Athletic Packet Deadline- August 9th- 4:00 P.M
 Tryout Date- August 10th @ end of dismissal
 Volleyball- Athletic Packet Deadline- September 30th- 4:00 P.M.
 Tryout Date- Week of October 10th- TBA
 Basketball- Athletic Packet Deadline- December 12th- 4:00 P.M.
 Tryout Date- Week of January 4th - TBA
 Track- Athletic Packet Deadline- February 17th- 4:00 P.M.
 Tryout Date- Week of February 27th – TBA


  • You must have a cumulative 2.0 grade point average from previous middle school years or previous semester whichever is greater.  Incoming 6th graders are already eligible for soccer and volleyball.

  • A student who reaches the age of 15 on or after September 1st will be ineligible for athletics at the middle school level.

  • There is only one girls team and one boys team for each sport:  Which means there will be a combination of 6th, 7th and 8th graders trying out for each team.  Both teams play on the same day, during the week at the same site. Girls games will start first this year followed by the boys game.

  • Practices are usually every day and could be before or after school depending on the coach or sport. This information can be requested further by contacting the Coach listed for each sport.  

  • There are no fees for being on the team other than providing the necessary safety equipment for your sport. (i.e. volleyball- knee pads, soccer-shin guards, etc.)
